Student Information Form

Appropriate documentation of your disability must be received by the Office of Student Accessibility (OSA) to qualify for services. Documentation guidelines can be found at:

The Office of Student Accessibility can begin processing your request as soon as you submit this Student Information Form. You will also need to provide the Office of Student Accessibility with documentation of disability. To do this you may upload your documentation now using the “Add Item” button.

You can also upload documentation later. To upload your documentation later, please follow these instructions: Log into Accommodate > Accommodations > Documents to upload.

Other options for providing your documentation later include: 

• Have your current provider of care fax your documentation to our office at 605.688.4504 (private fax).

The Office of Student Accessibility (OSA) is committed to protecting the privacy of your disability-related information in accordance with federal and state laws, including the Family Educational Rights and Privacy Act (FERPA).

- Your disability records are confidential and are only accessible to authorized OSA staff.

- Information about your disability will only be shared with university employees who have a legitimate educational interest, as permitted by law.

- OSA will not release your disability records to anyone outside the university without your written permission, unless required by law (such as in response to a court order or subpoena).

- Before authorizing the release of your information, you should understand what information will be shared, with whom, and for what purpose.

- In limited circumstances, OSA staff may share disability-related information with appropriate university personnel when there is a legitimate educational interest and doing so is necessary to support your access or ensure compliance with university responsibilities.

- You have the right to review your OSA file by providing reasonable advance notice.

By receiving services from OSA, you acknowledge that you have been informed of this confidentiality policy and understand that OSA may share relevant disability information with appropriate university officials who have a legitimate educational interest in supporting your access to university programs and services.
